Every guest at SUMS Place Society can work one-on-one with a case manager to sort out ID, health, income, and housing — at their own pace, on their own terms.
WHAT IS CASE MANAGEMENT?
One person, one plan, no wrong door.
Case management is one of our four pillars and how we support our guests beyond a safe bed and warm meal. A dedicated case manager gets to know each guest’s situation and helps them navigate the systems that can be hardest to face alone — from replacing a lost ID, to finding a doctor, to applying for housing.
There’s no set path. Guests choose what to work on, and their case manager helps makes it happen.

A guest lets staff know what they need assistance with, or is connected with a case manager during intake.

The case manager and guest map out the steps together, from paperwork, referrals, appointments and timelines.

Support
The case manager assists with booking appointments, making calls, and accompanying guests when it helps.

What Our Casework Provides

Losing ID can make everything else harder. We help guests replace foundational and photo identification.
Foundation documents: Canadian Birth Certificate, PR Card, Citizenship Certificate
Photo IDs: BCID, BC Services Card, BC Driver’s License
Record of name change: Marriage Certificate, Legal Name Change

Referrals and hands-on support across physical, mental, and cognitive health.
Physical health referrals: wound care, home health, doctor appointments
Mental health referrals: counselling, medications, ACT team
Substance use referrals: detox, treatment, recovery, ICM team
Cognitive impairment referrals: dementia, brain injuries, developmental disabilities
Family: help connecting with family and returning to place of origin, where appropriate
Accompaniment to critical appointments and advocacy
Preferred pharmacy support and medication assistance

Support finding and keeping a place to live, from shelter to long-term housing.
Long-term care referrals: seniors homes, assisted living, group homes
Supportive housing applications: VAT, SHR App, CAA referrals
Market housing searches: Craigslist, Facebook Marketplace
Landlord supports: negotiation and mediation
